Person centred planning.
PCP is a way for a person to be at the centre of and in control of planning for their life.

 

At Connections we try to make sure everything we do is person centred, this means we try to find out what each person who comes to our service wants from their lives and the support they need to have the life they want.

Some people want their own person centred plans, we have supported 15 people to have their own PCP's this year.

 

Some people are starting to realise their dreams, things like getting back in touch with old friends, going to college or work and planning to have their own home.

We have three PCP facilitators based at Connections, Jan who is one of the facilitators has written a book to help other facilitators plan well.

We have all had lots of training about PCP, this means we have lots of skills to use when we are supporting people to set up their Circle of support and have their own plan.

We think that PCP is the way forward for people with learning difficulties to have the kind of life they want, it is part of our job to support people to make this happen. It is very exciting to be involved in people's plans and to work with lots of different people, family, friends as well as staff to make things happen together
